Do you go heavy or just rep it out? Try going hard and heavy after warming up do sets of 5-6 reps to failure on the big compounds like flat bench and incline bench, I say ditch the ball and machine work and just stick to the barbell and dumbbells on benches. Like you already do, make sure to hit every angle when working out; incline, flat, and decline.
